The Loyalty Club is available to all patients. Our office is
dedicated to your oral health. As part of this dedication, we
recommend that our patients come in for a professional
cleaning every six months. As an incentive, we will give you a
FREE take home whitening kit after you complete the
minimum requirements*
So join the club and improve your oral health and whiten your
teeth!
*Loyalty Club Cards must have staff initials for each cleaning
*private and PPO patients are required to complete 3 consecutive regular cleanings or 3 perio maintenance cleanings
*families may combine their cleanings for the benefit of 1 whitening kit at which point new loyalty cards are started
